peTEP repair for rectus diastasis

A pre-aponeurotic endoscopic approach to repair rectus diastasis and associated small midline hernias through small access points.

Indications

  • Rectus diastasis with or without a small midline hernia
  • Umbilical or epigastric hernia associated with diastasis
  • Patients seeking a minimally invasive approach with small scars
  • Recurrence after previous open repair in selected cases

How it works

The peTEP approach creates a working space between the rectus muscle and the aponeurosis (pre-aponeurotic plane). This allows the surgeon to plicate the linea alba, repair associated hernias and, when needed, place a mesh, all through small ports.

Because the peritoneal cavity is not opened, the risk of bowel-related complications is reduced.

Assessment

A complete assessment includes clinical examination, dynamic ultrasound of the abdominal wall and evaluation of the pelvic floor and posture. A CT scan is used if larger defects are suspected.

Recovery

Early walking is encouraged. Abdominal-pelvic rehabilitation usually begins around the second postoperative month. Heavy lifting is avoided until the surgeon clears it.
